Diabetes Canada a r\u00e9cemment mis \u00e0 jour ses lignes directrices en mati\u00e8re de pr\u00e9vention et de prise en charge de la maladie. On y pr\u00e9sente notamment les plus r\u00e9centes donn\u00e9es cliniques afin de bien outiller les \u00e9tablissements de sant\u00e9 qui accueillent des personnes atteintes de diab\u00e8te ou \u00e0 risque de l\u2019\u00eatre.<\/p>\n<p><a href=\"https:\/\/www.diabetes.ca\/about-diabetes\/types-of-diabetes\">Diab\u00e8te <\/a>est une maladie chronique qui se traduit par l\u2019absence de production d\u2019insuline (type 1) ou l\u2019incapacit\u00e9 de l\u2019organisme \u00e0 utiliser correctement l\u2019insuline produite (type 2). Dans les deux cas, il en r\u00e9sulte une glyc\u00e9mie trop \u00e9lev\u00e9e, car l\u2019organisme n\u2019arrive pas \u00e0 utiliser le glucose comme source d\u2019\u00e9nergie.<\/p>\n<p>En 2016, le pourcentage de Canadiens ayant re\u00e7u un diagnostic de diab\u00e8te de type 2 atteignait 7 %. Forme la plus fr\u00e9quente de la maladie (environ 90 % des cas), le diab\u00e8te de type 2 se manifeste g\u00e9n\u00e9ralement chez les personnes de plus de 40 ans. Son impact se fait donc beaucoup sentir dans les centres de soins de longue dur\u00e9e. Par exemple, en Ontario, pr\u00e8s de 28 % des r\u00e9sidents de ces centres en sont atteints. C\u2019est pourquoi les services alimentaires de ces milieux doivent absolument conna\u00eetre et comprendre les meilleures pratiques \u00e0 adopter.<\/p>\n<h3>Principaux \u00e9l\u00e9ments mis \u00e0 jour<\/h3>\n<p><a href=\"https:\/\/www.dietitians.ca\/\">Publi\u00e9es par Diabetes Canada<\/a>, les Lignes directrices de pratique clinique pour la pr\u00e9vention et le traitement du diab\u00e8te au Canada <a title=\"diabetes guidelines\" href=\"http:\/\/guidelines.diabetes.ca\/cpg\">sont mises \u00e0 jour tous<\/a> les cinq ans. Elles donnent acc\u00e8s aux donn\u00e9es cliniques probantes les plus pertinentes et actuelles sur le sujet<\/p>\n<p>On y trouve d\u2019ailleurs un chapitre sur la prise en charge du diab\u00e8te chez les a\u00een\u00e9s qui est tr\u00e8s pertinent pour les services alimentaires. Voici les trois points \u00e0 retenir de cette section :<\/p>\n<ul>\n<li><a href=\"http:\/\/guidelines.diabetes.ca\/cpg\/chapter37\">Chez les a\u00een\u00e9s<\/a> , le diab\u00e8te se manifeste diff\u00e9remment de chez le reste de la population et doit donc \u00eatre trait\u00e9 en cons\u00e9quence. Cela est particuli\u00e8rement vrai pour les gens qui ont une d\u00e9pendance fonctionnelle ou qui sont fragiles, atteints de d\u00e9mence ou en fin de vie. Pour \u00e9viter le surtraitement, il faut recourir \u00e0 des strat\u00e9gies personnalis\u00e9es.<\/li>\n<li>Chez les personnes \u00e2g\u00e9es atteintes de diab\u00e8tes et d\u2019autres comorbidit\u00e9s ou chez celles qui sont fragiles, les strat\u00e9gies privil\u00e9gi\u00e9es doivent viser exclusivement \u00e0 pr\u00e9venir l\u2019hypoglyc\u00e9mie; les cibles glyc\u00e9miques doivent donc \u00eatre moins strictes.<\/li>\n<li>Chaque plan de soins doit \u00eatre adapt\u00e9 en fonction de la personne; ce qui fonctionne pour quelqu\u2019un pourrait ne pas constituer le meilleur traitement pour une autre personne.<\/li>\n<\/ul>\n<h3>Miser sur des soins individualis\u00e9s<\/h3>\n<p>Chez les personnes diab\u00e9tiques, tous les aspects des soins et de l\u2019alimentation doivent \u00eatre examin\u00e9s au cas par cas. Il est \u00e9galement crucial d\u2019\u00e9viter de surtraiter les personnes atteintes de diab\u00e8te en restreignant leur di\u00e8te, puisque cela pourrait entra\u00eener des complications.<\/p>\n<p>M\u00eame si les di\u00e8tes th\u00e9rapeutiques sont utilis\u00e9es depuis longtemps, les \u00e9tudes tendent de plus en plus \u00e0 montrer que, chez les diab\u00e9tiques, ces di\u00e8tes risquent de mener \u00e0 une diminution de la prise alimentaire, une perte de poids involontaire et la malnutrition.<\/p>\n<p>M\u00eame son de cloche chez Les Di\u00e9titistes du Canada, o\u00f9 l\u2019on pr\u00e9cise que la r\u00e9duction de l\u2019apport alimentaire et la perte de poids peuvent entra\u00eener un risque accru de morbidit\u00e9 et de mortalit\u00e9.
